Remove vulkan_fuchsia_extras.h

This change ensures our Vulkan headers match the upstream Khronos ones
we're using (currently 1.2.189).

Fuchsia's Vulkan extension definitions are now part of vulkan_fuchsia.h,
except for VK_FUCHSIA_buffer_collection, but we don't implement that at
the moment. Other definitions in vulkan_fuchsia_extras.h with _TEMP_ in
their name have official names now without that infix, and new numeric
values.
